- Collect and prepare the documents below to upload to the enrollment website.

- Child's Immunization Record
- Emergency Contacts
- School Records ( if applicable)
- Parent/ Guardian's Photo Identification
- Proof of Residency - Only a Lease Agreement or Utility Bill is accepted (Light, Water, or Gas Bill)
